New Patient Registration

The catchment area for the practice is TS24, TS25, TS26 and the inner boundaries of TS27. please ask at reception to check if your address is within our catchment area.

To register as a new patient you will need to complete a registration form which can be obtained from the receptionist. You then need an appointment with the practice nurse for a new patient check.  This is to enquire about your family history, any medication you take and any other problems we should be aware of.  Please note without this appointment you will not be accepted onto our list. 

Change of Name/Address/Telephone Numbers 

If you change your name, move house or change telephone numbers, please let us have your correct details in writing as soon as possible.
